Leaving the hospital?

Back home, you have a role to play in facilitating your recovery. And even more so if you're over 75!

When you return home, follow this advice.

Tips to facilitate your recovery

(unless otherwise advised by your doctor)

Get moving and be active

  • Get moving every day, within your capabilities.
  • Maintain your habits and engage in your daily routine as much as possible consistent with your capabilities.
  • Participate in physical activities in your home environment or offered in your community.
  • Ask for an exercise program tailored to your abilities.
  • At the start of the day, get dressed and begin your routine.
  • Do minor interior and exterior work to the best of your ability.

Make sure that you eat and drink adequately

  • You need to eat to regain your strength. Eat at regular times. Make sure you have a menu adapted to your needs.
  • Ask for help with meal preparation when you need it.
  • If necessary, have your groceries delivered.
  • If necessary, use catering services or Meals on Wheels.
  • Keep fresh water available and accessible at all times. If you don't have any restriction for your liquid intake, drink the whole glass of water when taking your medication.
  • Wear your dentures, if you have them, and make sure you receive the dental care you need to be able to continue eating without problems.

Prevent constipation

To prevent constipation (3 days or more without a bowel movement):

  • Eat, drink, and stay active.
  • Inform a relative or health-care professional if you think you might be constipated.

Rest

  • Respect your need for sleep and rest.
  • Do not take long naps.
  • Have an evening routine to help you regulate your sleep.
  • Be active during the day.

Prevent disorientation and confusion

  • Crossword puzzles, reading, puzzles, card games...any activity that keeps your mind active!
  • Wear your hearing aids and glasses, if applicable.
  • Use a calendar to keep track of the days of the week.
  • Record your activities and appointments in one place. 
  • Keep a clock nearby to guide you through your daily routine.
  • Plan your routine and activities in your schedule.
  • Ask to be accompanied on certain outings and for your medical appointments.
  • Open the curtains during the day.
  • Learn new things.
  • Look at photos to bring back fond memories.

Keep track of your health

  • Inform your family and health-care professionals of any changes in your health or in your ability to carry out your usual daily activities.
  • Pay attention to your energy level and your capabilities. Note any changes, so that you can pass the information on to someone you trust.
  • Make sure you follow up with your medical appointments (family doctor, optometrist, dentist, physiotherapist, ENT specialist, etc.).
